For all we know Eugenia could throw tantrums or just be plain mean. We don't know. Her mom is trying to deal with 2 children with their own issues and with a husband working long hours. Maybe she's guilty of making poor parenting choices or maybe she has her own issues.
I have no idea. I think the general consensus is luck + genetics that allow her to have gotten that low without dying, but it's a rare occurrence.
