Put the contact as the most recent doctor you shadowed or the one that you spent the most time shadowing. I think it looks a bit weird if you put yourself as the contact. The format looks good! I don’t think you even need an experience description, maybe just a brief description of what you observed under each doctor

IMO shadowing should not be your most meaningful activity, you literally just followed someone who was doing their job.
